About Us: Mission Grace Home Care is a newly opened, RN-owned and operated assisted living home located in North Phoenix. Licensed for 10 residents, we offer a supportive, home-like environment focused on quality care and dignity for seniors. We specialize in assisting with activities of daily living while promoting independence, medication management and hospice, respite and memory care services.

We are seeking compassionate, reliable Caregivers or CNAs to bridge to Caregivers, to join our team! You'll work under the direct supervision of a Registered Nurse in a small, high-quality care setting.

Responsibilities:

1. - Assist residents with activities of daily living (bathing, grooming, toileting, dressing, etc.)
2. - Medication administration
3. - Support residents with mobility and transfers
4. - Monitor and document changes in resident health or behavior
5. - Provide companionship and emotional support
6. - Light housekeeping and meal preparation
7. - Communicate effectively with residents, families, facility manager and interdisciplinary team members

Qualifications:

1. - Previous caregiving experience preferred (in hospital, nursing home or assisted living home setting)
2. - Caregiver or CNA certification
3. - CPR/First Aid certification
4. - Ability to pass a background check and drug screening
5. - Compassionate, dependable and team-oriented
