Overview: We are seeking a compassionate and skilled Nurse Caregiver to join our team. The ideal candidate will have a strong background in nursing care and be proficient in utilizing medical equipment such as a Hoyer lift for patient transfers. They should possess exceptional communication skills and be able to provide assistance with activities of daily living, including bowel movement assistance and skilled transfers. Responsibilities: Patient Care: Provide compassionate and skilled care to patients in accordance with their individual care plans and medical needs. Utilization of Medical Equipment: Proficiently operate and utilize medical equipment, including but not limited to Hoyer lifts, to safely transfer patients with mobility limitations. Assist with Bowel Movements: Provide assistance with bowel movements in a respectful and dignified manner, ensuring patient comfort and hygiene. Skilled Transfers: Perform skilled transfers of patients from beds to wheelchairs, chairs, or other surfaces using appropriate techniques and equipment to prevent injury to both the patient and caregiver. Monitoring and Reporting: Monitor patients&#039; vital signs, symptoms, and overall condition, promptly reporting any changes to the nursing team or healthcare provider. Medication Management: Assist with medication administration as directed by healthcare professionals, ensuring accuracy and compliance with medication protocols. Personal Care: Assist patients with activities of daily living, including bathing, dressing, grooming, and toileting, while maintaining their dignity and privacy. Emotional Support: Provide emotional support and companionship to patients and their families, fostering a supportive and nurturing environment. Documentation: Maintain accurate and thorough documentation of patient care activities, including assessments, interventions, and outcomes, in accordance with facility policies and procedures. Qualifications: Licensed Practical Nurse (LPN) or Certified Nursing Assistant (CNA) certification: Must hold a valid LPN or CNA certification, demonstrating competency in nursing skills and patient care. Experience: Previous experience working as a nurse caregiver in a healthcare setting is preferred. Hoyer Lift Proficiency: Demonstrated ability to safely operate and utilize a Hoyer lift for patient transfers is essential. Communication Skills: Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and healthcare team members. Compassion and Empathy: Strong interpersonal skills with a compassionate and empathetic approach to patient care. Physical Stamina: Ability to perform physical tasks associated with patient care, including lifting, transferring, and assisting with mobility. Attention to Detail: Keen attention to detail and the ability to follow instructions accurately, ensuring patient safety and well-being.   3. Documentation Management: Maintain accurate and up-to-date records of client schedules, caregiver assignments, and any changes or modifications, ensuring compliance with regulatory standards.
  4. Client Engagement: Provide exceptional customer service to clients and their families, offering support, guidance, and reassurance throughout the scheduling process.
  5. Training and Development: Stay informed about industry trends, best practices, and technological advancements related to scheduling and care coordination, participating in training sessions and professional development activities as needed.

Qualifications:

  • Previous experience in scheduling or care coordination, preferably in a home care setting.
  • Strong organizational skills with the ability to manage complex schedules and competing priorities effectively.
